Agreement for the Placement – How to Fill It In?

How to Correctly Complete the Placement Agreement?

What should I fill in?

  • Fill in your personal information (2nd half of page 1).
  • Add the period during which you will complete the placement (also on page 1).

What should the company/organisation fill in?

  • Company/organisation details (1st half of page 1).
  • Date, stamp, place of signing (2nd half of page 2).

How to Correctly Submit the Placement Agreement?

  • Prepare 2 copies of the Agreement — completed and signed (by the employer).
  • Deliver the Agreements to the office of Mgr. Martin Poledna, room 435 at the Rectorate (5th door on the right).
  • Submit the electronic version to the Submission Folder (see the section Submission Folder for Electronic Signature of the OP Agreement in this Interactive Outline).

Submission Deadline

  • The completed Agreement (for signature) must be submitted by 31 March 2026.
  • Late submissions are possible only with prior approval.
  • In case of any issues, resolve them immediately — consultation (online, etc.).

ATTENTION — the signed Agreement (signed by both parties) must be uploaded to the Submission Folder by 31 March 2026.
